Financial, Cancellation & Refund Policy

Practice: Elite NP Clinic
Elite NP Clinic is committed to providing high-quality medical care while maintaining transparent financial and scheduling policies. This policy applies to both insured and self-pay patients unless otherwise stated.
Payment for Medical Services
Payment for applicable copays, deductibles, coinsurance, self-pay services, and other patient-responsibility amounts is due at the time of service unless other arrangements have been approved by Elite NP Clinic.
Elite NP Clinic accepts both insurance and self-pay patients.
Patients are responsible for providing accurate and current insurance information before receiving services.
Insurance Patients
As a courtesy, Elite NP Clinic will submit claims to participating insurance companies when applicable.
Verification of insurance coverage or benefits is not a guarantee of payment by your insurance company.
Patients are ultimately responsible for:
Copays
Deductibles
Coinsurance
Non-covered services
Services denied by their insurance carrier
Amounts determined by their insurance plan to be patient responsibility
If your insurance company processes your claim differently than initially estimated, your account will be adjusted accordingly.
Self-Pay Patients
Self-pay patients are responsible for payment according to the rates provided by Elite NP Clinic.
Payment is generally due at the time services are rendered unless another payment arrangement has been approved by the practice.
Patients may request information regarding anticipated charges before receiving scheduled services.
Medical Services and Refunds
Fees for medical services that have already been provided are generally non-refundable.
Medical care is individualized, and Elite NP Clinic cannot guarantee a specific medical outcome, response to treatment, symptom improvement, weight change, laboratory result, medication response, or other clinical result.
Dissatisfaction with the outcome of a medical service does not automatically entitle a patient to a refund.
Nothing in this policy limits any rights or remedies available to a patient under applicable law.
Overpayments and Billing Errors
If a patient’s account reflects a confirmed overpayment after insurance processing and account reconciliation, the overpayment may be refunded or applied to an outstanding balance when permitted by applicable law and payer requirements.
Refunds resulting from billing errors or insurance adjustments will be processed after the account has been reviewed and the appropriate amount has been confirmed.
Appointment Cancellation Policy
Elite NP Clinic requires patients to provide at least 24 hours’ notice when canceling or rescheduling an appointment.
Providing adequate notice allows us to offer the appointment to another patient who may need care.
Same-Day Cancellations and No-Shows
Appointments canceled with less than 24 hours’ notice are considered late or same-day cancellations.
A $75 fee will be charged for:
Cancellations made with less than 24 hours’ notice
Same-day cancellations
Failure to appear for a scheduled appointment (“no-show”)
Cancellation and no-show fees are generally the patient’s responsibility and are not billed to insurance.
Elite NP Clinic reserves the right to waive a cancellation or no-show fee in its discretion when appropriate, including certain emergencies or extenuating circumstances.
Late Arrivals
Patients who arrive late may have their appointment shortened or may need to be rescheduled depending on provider availability and the amount of time remaining for the scheduled visit.
If the appointment must be rescheduled because of a late arrival, applicable cancellation policies may apply.
Prepaid Services and Deposits
If a medical service requires a deposit or advance payment, any applicable cancellation and refund terms will be communicated to the patient before payment.
Deposits or advance payments may be non-refundable when clearly disclosed at the time of booking or purchase, subject to applicable law.
Prescription Medications
Prescription medications, including medications dispensed or obtained through a pharmacy, generally cannot be returned or refunded once dispensed, except where otherwise required by law or permitted by the dispensing pharmacy.
Medication effectiveness and individual response cannot be guaranteed.
Laboratory and Third-Party Charges
Certain laboratory tests, imaging services, medications, diagnostic services, or other services may be performed or billed by independent third parties.
Patients may receive separate bills from these organizations. Elite NP Clinic is not responsible for refund policies, billing practices, or charges established by independent third-party providers.
Patients should contact the applicable third party regarding disputes involving charges billed directly by that organization.
Charge Disputes
If you believe you have been charged incorrectly, please contact Elite NP Clinic so that we can review your account.
We encourage patients to contact our office regarding billing concerns before initiating a payment dispute or chargeback so that we have an opportunity to investigate and resolve the matter.
Outstanding Balances
Patients are responsible for balances assigned to them after insurance processing or for unpaid self-pay services.
Outstanding balances may affect the ability to schedule non-urgent future appointments, subject to applicable law, payer contracts, continuity-of-care requirements, and Elite NP Clinic policies.
Policy Changes
Elite NP Clinic reserves the right to modify this Financial, Cancellation & Refund Policy. Updated policies will become effective when posted or otherwise communicated to patients, as permitted by law.
